• Please read your insurance handbook before your visit and remember to bring your insurance card to your appointment. It is your responsibility to ensure that your child is covered by your insurance plan, and that one of our doctors is listed as your child's primary care physician.
• If your insurance will not pay for your visit, it is your responsibility to make all payments due.
